3. Insufficient safety quality of employees. Insufficient safety education and training for employees, safety management personnel and grassroots operators have not received training or insufficient training hours, and do not have the corresponding safety production knowledge and job ability; The quality of employees is low, the knowledge is outdated, and the concept is backward, resulting in poor safety awareness and a large number of unsafe behaviors; The safety education and training of foreign employees are ignored.
4. loose management of hazardous chemicals, hazardous processes and major hazard sources, inadequate monitoring measures, insufficient risk identification, unclear control methods, insufficient prevention and emergency treatment, and backward predictive management methods.
5. The supervision, inspection and assessment of management personnel are insufficient, there are prohibitions, orders, illegal behaviors, and special operation safety management is not in place.
6. Hidden dangers are not thoroughly investigated and control measures are not appropriate.
7. New construction, renovation projects or new equipment of enterprises fail to carry out "three simultaneous" safety facilities as required, resulting in congenital deficiency of safety production conditions.
8. Insufficient investment in safety funds, insufficient safety education and training, inadequate personal protection, insufficient safety facilities, and failure to provide funds for the investigation and management of hidden dangers of accidents have led to accidents.
9. Failure to establish safety production record files is not conducive to timely, comprehensive and systematic grasp of the safety production situation of enterprises and timely reflection of safety production trends; It is not conducive to analyzing the risk factors in safety production and making safety management decisions. 10.
